P2C 50 Anniversary – Share Your Story!

I am submitting a story (collectively with any related images, video, audio or written material) to the non-profit organization of Power to Change Ministries via the organization’s online form (www.p2c.com/share50). The submitted story is given to the organization on the following conditions:

1. I represent and warrant that the submitted story I submit is from my personal experience; that the submitted story is true and not misleading; that I have the exclusive, unconditional right and authority to submit the Story to Power to Change Ministries upon the terms and conditions set forth herein.

2. I hereby grant (and release to) Power to Change Ministries the ability to publish my story for use of advertising or publicity purposes, and otherwise use the submitted story (or any portion or derivation thereof) in web, print, video, audio, and all other media (including, without limitation, any and all social media) and in any internet, radio, television, or live stage program. I agree that I will not be entitled to any compensation from Power to Change Ministries. Power to Change Ministries also cannot guarantee that other parties will not use the submitted story, and Power to Change Ministries, and its affiliates, are not responsible or liable for any use, misuse or misappropriation of the story.

3. I have retained at least one copy of the story, and I release Power to Change Ministries from any and all liability for loss or other damage to the copies of the submitted story to Power to Change Ministries hereunder.

4. I have read and understand this release and license and no oral representations of any kind have been made to me. This release and license states our entire understanding with reference to the subject matter hereof.
